POSITION:  Medical Case Manager 

REPORTS TO:  Care Program Managers 

SALARY: $45,000

JOB SUMMARY:

Under the supervision of the Care Program Manager(s), this person is responsible for the provision of medical case management oversight, including adherence to medical protocols, risk reduction and health education services, and outreach to targeted populations, i.e. bringing clients out of care into care.  The goal of the Medical Case Manager is to help individuals living with HIV to access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to prescribed medical treatment. The Medical Case Manager meets regularly with the care team to monitor program objectives and other issues pertinent to the successful completion of various goals. The position is also responsible for attendance at regular staff meetings.

LICENSES AND CERTIFICATIONS

A valid Michigan Vehicle Operator’s License and Michigan Department of Health and Human Services Certification as a Case Manager (or ability to obtain within three months of hire or as MDCH training schedule allows).

EDUCATION

At minimum, a Bachelor’s Degree in Social Work or equivalent; medical training or work in the medical field and experience with HIV/AIDS will be helpful. 

EXPERIENCE

Experience working with chronically ill, diverse populations, experience in HIV/AIDS or the willingness to become facile in all aspects of HIV/AIDS.

Lansing Area AIDS Network (LAAN) is an AIDS Service Organization founded in 1985, providing critical services and support to those living with HIV in the greater capital region. LAAN also provides necessary prevention and harm reduction services to help prevent the further spread of the virus. Our team is dedicated to this mission, and to responding effectively, knowledgably, and compassionately to the challenges brought about by the HIV/AIDS epidemic.

JOB SUMMARY:

The Program Coordinator is a full-time position providing administrative support and clinical supervision within the Behavioral Health Department. Responsibilities include developing and implementing policies and procedures; supervising and training team members and interns; implementing, monitoring, and advancing quality and performance outcomes; ensuring compliance with relevant standards of care, laws, and regulations;  providing affirming evidence-based therapies including trauma-informed cognitive behavioral therapy, motivational interviewing, and others; and developing infrastructure to provide, document, and bill for mental health services.

REQUIRED QUALIFICATIONS

  • Licensed (State of Michigan) Master Level Social Worker (LMSW)
  • Basic knowledge of HIV/AIDS
  • Personal and/or professional experience with the LGBTQ+ community
  • Cultural competence, strong demonstrated commitment to diversity, equity, and inclusion. 
  •  

RESPONSIBILITES:

·         Provides evidence-based therapy, diagnoses and treats mental health disorders, and refers clients to specialists and/or other community resources,  as necessary.

·         Ensures timely, accurate, and complete documentation at the patient/client and aggregate levels in systems and formats required for assigned program areas.

·         Participates in community needs assessments, program planning, and resource allocation activities; develops, evaluates, and adapts programs in response to identified needs and opportunities.

·         Provides clinical supervision to mental health therapists, limited licensed social workers, and interns.

·         Prepares materials, compiles reports, emails, letters, and other documents to ensure that funder requirements are met, including service goals and performance measures.

·         Implements quality and performance improvement efforts. Responsible for site visit preparation and audit processes in assigned areas.

·         Develops, reviews, and updates policies, procedures, and practices to promote effective, client-centered care, productivity, fiscal responsibility, safety, risk management, employee engagement, and satisfaction.

·         Upholds compliance efforts. Ensures confidentiality and adherence to privacy practices in accordance with agency policies, HIPAA, and any relevant laws, regulations, and contractual obligations.

·         Prepares, gathers, and reports data for various constituencies which may include 

·         organization leadership, funders, staff, clients, and the public.

·         Represents the agency at community meetings and venues.

·         Strategizes outreach activities to promote services and education in the community.

·         Supervises, mentors, evaluates, and supports direct reports and provides management coverage and oversight to other team members as needed.

SCHEDULE & WORKING CONDITIONS

This position is full-time, predominantly during business hours Monday through Friday, and will require some evening and weekend hours. The position is based in Lansing. Work is generally performed within the office environment and includes significant in-person, phone, and email contact with staff, patients/clients, funders, and the general public. This position reports to the Executive Director.
